South Boston's Healthy Aging and Wellness Collaborative
Every three years, Tufts Medical Center conducts an extensive Community Health Needs Assessment (CHNA) to identify the health and social needs of the communities it serves. South Boston Neighborhood House was identified as a grant recipient for funding years 2026-2029 to lead a collaboration that supports physical and mental well-being for older adults throughout South Boston.
This initiative, funded by Liberty Mutual Foundation, enabled a local collaboration to develop a climate-responsive recovery plan, fostering sustainability and resilience in the aftermath of a major climate event. Collectively, these efforts enabled SBANP to expand its reach within South Boston, connect with some of our most vulnerable residents -- including older adults (55+) and low-income families -- to share critical resources to support a safer, healthier, and more resilient community.
Since 2005, Procter & Gamble/Gillette has provided grants to South Boston Association of Non Profits to address the problem of substance-use disorder in the neighborhood. This year, P&G/Gillette has committed $150,000 in funding for the Gillette Great Starts grant program to support substance-abuse treatment and prevention programs in South Boston.

This series held space for intentional conversations about racism and anti-racism. Each conversation revealed unique stories, feelings, and reactions from locals and civic leaders about court-ordered desegregation (busing), and explored the lasting impact from the mid-1970s through today.

As a follow-up to our 2021 Courageous Conversations, Bob Monahan has represented the SBANP on a citywide Desegregation and Busing working group coordinated by Lew Finfer, Director and Community organizer for MA Action for Justice.
