South Boston's Healthy Aging and Wellness Collaborative
South Boston's Healthy Aging and Wellness Collaborative, made possible by Tufts Medical Center, takes a community-centered approach to increasing access and services for older adults. The Collaborative integrates the collective strengths and expertise of three members of the SBANP: Fourth Presbyterian Church, South Boston Neighborhood Development Corporation (SBNDC), and South Boston Neighborhood House (SBNH) to provide an expanded, coordinated network of services for older adults in South Boston.

All programming promotes resilience and social connection. Specific programs include Peace by Piece group sessions, traditional "chair" yoga for older adults, strength-training exercise classes, educational workshops, and more. The Collaborative offers evidence-based health education, nutrition and fitness programs, and regular health screenings to identify risk factors and encourage healthy lifestyle changes - all at no cost to participants.

Our efforts focus on all older adults in South Boston, to help them maintain both physical and mental well-being while staying engaged in their community.
